Getting Started

Talk to your physician about medical cannabis and educate them about the program. Have your physician complete a Physician Certification Form and mail to IDPH.

Begin the Illinois Medical Cannabis Application. Patients may submit an Online Application, or print and mail a Paper Applications to IDPH. Applications must also include:

  • Proof of identity and age as evidenced by a valid government-issued photo ID.
  • A photograph that is less than 30 days old, taken against a plain background.
  • Proof of residency, any two of the following items:
    • Pay stub
    • Voter registration card
    • Valid Illinois license or state ID
    • Bank statement
    • Mortgage or lease
    • Insurance policy/medical claim
    • Tuition invoice
    • Utility bill
Application Fee: $100 or $50 reduced fee for veterans and SSDI recipients.

To satisfy the background check requirement, visit an Illinois licensed Live Scan Vendor to get fingerprinted, and fill out a Fingerprint Consent Form. Be sure to include the Transaction Control Number (TCN) that the Live Scan Vendor will provide.
